Who is this service for?

This service is available for individuals over 18 who may need short-term rehabilitation to regain their independence after a hospital stay or to avoid hospital admission.

Examples of when you or someone you care for may need this service include:

  1. A sudden deterioration in function requiring advice, education or treatment in your own home to prevent admission to hospital with the aim of returning to your previous level of function and independence.
  2. If you, or the person you care for, requires therapy intervention in a timely manner on discharge from hospital and have weekly goals which are achievable in a community setting.


Types of Intermediate Care:

  1. Home-based: Provides rehabilitation in your usual place of residence. Working alongside healthcare professionals to agree goals and the type of support you need to regain your function and independence.
  2. Bed-based: Provides rehabilitation as part of a temporary stay in a residential or nursing home setting. Working alongside healthcare professionals to agree goals and the type of support you need to regain your function and independence, with the aim of returning to your usual place of residence.


What does the service do?

We have a team of Physiotherapists, Occupational Therapists, Nurses, Paramedics and Support Workers. who help people living with frailty, multiple long term health conditions, and/or complex needs to stay independent, healthy, and at home for as long as possible.

Ways in which we can support:

We can support you (or the person you care for) to do things for yourself, promoting independence and building confidence so you may need less support in the future. This could include:

  1. Undertaking everyday activities such as washing, dressing and meal preparation.
  2. Participating in exercise programmes, mobility practice and transfer practice.
  3. Providing advice, education and equipment.


What to expect?

  1. This is a short-term rehabilitation service. The length of input varies person to person and depends on individual needs.
  2. An initial assessment will be carried out within 48 hours of receiving the referral which will identify needs and support required.
  3. Regular reviews are undertaken by the team and can be done face to face or via telephone. Including discussing progress and plans.
  4. Support visits are not allocated a time or timeslot, and times can vary from visit to visit.


Who does this service work with?

GP Practices, Hospital Discharge Teams, NHS 111, Ambulance Services and Social Care Teams. As well as working alongside Mental Health, Housing Sector, Voluntary Sector, and Community Teams.

When is the service delivered?

The service operates 7 days a week, from 08:00 to 20.00.

Ongoing support:

We may discuss referrals to other community health and social care or voluntary sector providers, to support your ongoing care and rehab. This can include referrals to Health and Adult services or longer-term community therapy teams.

Which area is covered by this service?

The service is delivered across Scarborough and Ryedale Community Services. There is no charge for this service.
